Q. At what age am I covered / eligible for Medicare Supplement?
A. Most folks become eligible for Medicare benefits, and Medicare supplement policies, when they reach 65 years of age.
Certain other factors, such as disability, may qualify a person for Medicare prior to reaching age 65.

Q. How do I submit a Medicare Supplement claim?
A. To service a Medicare Supplement claim, all we need is the following:
A copy of your Medicare Explanation of Benefits form you received from Medicare
("EOMB") with your full name and Bankers policy number clearly
written on it. Most people choose to write it on top, where there isn't a
lot of Medicare preprinted information. That's all we need to process your
claim!

Q. I haven't received benefits on my recent Medicare Supplement claim. What should I do?
A. Our claim service is one of the fastest in the industry. If you
haven't heard from us within two weeks of receiving your EOMB, it is because
the claim never reached us. Just send us a copy of the EOMB and we'll get
right to work.

Q. How do I file a claim on my Hospital Confinement Indemnity policy?
A. You need only send us a copy of the Hospital bills showing the dates of confinement and the diagnosis.

Q. How do I receive benefits under my Long-Term Care, Short-Term Care, Facility Care, or Home Health Care coverage?
We want to help you use the benefits available under the terms of your policy. Please call 1-800-621-3724 and ask for our specialized Intake Team at to learn more about receiving benefits prior to filing a claim. You or your authorized representative will learn about qualifying for policy benefits, claim filing procedures, and how to find a qualified provider in your area.
Completing a Claim Form
Once your care begins, you will need to complete the "Long-Term Care and Short-Term Care Claim Form." Click Here to go to the form download page.
Please keep the following items in mind as you complete the claim form.
- Make certain to provide as much detail as possible to each of the 10 questions. This includes providing current addresses and telephone numbers for questions 1, 2, 6, and 7. Providing incomplete information may lengthen claim processing time.
- Question 7 allows you to describe a facility confinement under section "A" and/or care services under section "B." Make sure to provide complete responses regarding services you have received.
- Feel free to attach additional pages if you need more room to respond to any question.
- Before returning the document to our company, collect and attach copies of all available bills for services received, care service notes for a non-facility claim, and proof of legal authority of any person that represents you, if available.
- Return your completed claim form and all attachments to Bankers Life and Casualty Company via fax (1-312-396-5952) or by mail (PO Box 1902, Carmel, IN 46082-1902). The claim department will contact you within ten to fifteen business days of its receipt of your documents.
