Long Term Care/Home Health Care/Short Term Care Policies
California: Long Term Care and Home Health policies that provide non-facility care must and do provide "Home Care" which is broader than "Home Health Care," the term used in most other states.
Samples of individual LTC policy forms offered in California are available to prospective applicants upon request.

Idaho: The following policy forms are available: GR-N130, GR-N240, GR-N250, GR-N270, GR-N280, GR-N400 and GR-N410

Not all plans are available in all states
Maine: All plans are available to those persons eligible for Medicare because of disability.
Maryland: Medicare supplement insurance products are available to persons eligible for Medicare because of disability.
North Carolina: Medicare supplement premiums are based on attained age, meaning they will increase each year.

Annuity policy forms numbers are: LA-02P, LA-03D, LA-06T,
LA-07C, LA-08N, LA-69A.
Life policy form numbers are: L-14U, L-100(80), L-15P, L-18G,
L-19A, L-20A, L-5Z1 and L-62J.
